Accessing Services
We are committed to gathering all relevant information to ensure the highest quality of service. Our services are available to adults who receive funding through Persons with Developmental Disabilities (PDD), through Family Managed Services. Our services are available to children and youth who receive funding through Family Supports for Children with Disabilities (FSCD), Delegated First Nation Agencies (DFNAs) or Children & Family Services (CFS). What to Expect During the Intake Process
Collaborative Engagement: The intake process involves active participation from the individual, their family or legal guardians, circle of support, and relevant professionals to ensure full disclosure for the safety of individuals accessing service and for staff.
Comprehensive Assessment: A range of current and past assessment tools will be utilized to evaluate the individual’s needs, strengths, goals and potential areas of risk, including:
- Previous assessments and evaluations
- Past support plans
- Educational or Individualized Program Plans (IPPs)
- Medical documentation and reports
- Additional assessments (e.g., occupational therapy, speech-language pathology)
Informed Consent: Review of the individual’s legal, cultural and human rights along with rights as an individual accessing HNCLAS services
Orientation to Services: An introduction to applicable policies and procedures and services provided
Following the initial assessment, the team co-creates with the individual and their support network, strategies and a timeline to assist individuals in completing a smooth transition plan. The orientation process begins as soon as the individual starts receiving service.
It is the goal of HNCLAS to deliver consistent, high-quality care that supports the individual in achieving their goals and living a meaningful life. This process is reviewed at three months in the first year and annually thereafter, or more frequently as needed, to ensure the care remains appropriate and effective.
